Allergies or Special Considerations
I give permission for photographs and/or digital images to be taken of my child and understand that they are the property of Fellowship Baptist Church and may be used by the church for church purposes and publications including its website.
In the event of an EMERGENCY, I hereby authorize the church or its representatives to initiate any medically necessary care on my behalf in the event of my incapability to present myself for such care.
Participant Agreement and Waiver
I, the undersigned, agree to release, discharge, and hold harmless Fellowship Baptist Church, its employees, agents and members from any and all claims or demands due to personal injury, illness, or death as well as any property damage sustained of any nature which may be incurred by the child listed above while participating in the above described event or activity; except to the extent any claims for damage or loss arise from the negligence or willful misconduct of Fellowship Baptist Church, its employees, agents and members. I also agree to hold harmless and to indemnify Fellowship Baptist Church as well as its employees, agents or members for any liability or expenses sustained by the church as a result of my child participating.
